Once you have a slip knot on your hook, you can begin making chains.
Yarn over.
Um, yeah, yarn over.
Twist the hook slightly so that it faces downward and pull the yarn through the loop on your hook.
Keep pulling.
And, you have your first chain!
Continue to yarn over and pull the yarn through the following chains.
This part may take a lot of practice to get the chains to be uniform and large enough to fit the hook back into. As you are going, if the hook won't fit you can loosen the yarn by pulling it back over the shaft of the hook.
